Qualified Developmental Disability Professional (QDDP)
Looking for Contractors only, at this time.
Supervises waiver services to ensure health and safety.
Safeguards individuals by overseeing residential sponsors as well as ensuring documents and services are in accordance with DBHDS regulations.
Write and implement effective person-centered plans (PCP) and Behavior Management Plans for each individual being served within caseload.
Read and review progress notes to ensure goals are being met and to ensure all documentation is current and in compliance.
Communicates with Support Coordinators regarding Individuals.
Attends in-house meeting as well as community meetings and conducts monthly visits to individuals.
Assist in ensuring that residential sponsors are trained adequately to provide the proper services in the homes.
Provide and gather new information concerning the individual’s health, safety, future goals and achievements.
Observes individuals, reviews data and progress, and revises programs based on individual need and performance.
The QDDP is responsible to build maintain rapport with all parties involved in the support of individuals served.
Performs other duties as deemed appropriate and as assigned by Executive Director.
Knowledge, Skills and Abilities
The following are the essential functions of this position: Obtains and retains certification: CPR/AED, First Aid and Therapeutic Intervention, BS/BA Degree from 4-year college; QDDP status verified; five years related experience in the field of intellectual disabilities and supervision.
Computer skills and receptionist duties. Listens to and responds to residential sponsor and individual in a timely manner, providing follow-up. Participate in Admission and Discharge meetings for individuals in assigned homes. Conduct regular audits and follow up with residential sponsors on any errors or discrepancies in data collection. Complete required daily, monthly, quarterly, semiannual and annual program documentation and other required reports in a timely manner. Valid VA driver's license with reliable transportation; Willing to work flexible hours-some evening and weekend. Must have strong organizational skills.
Training status for QDDP - Staff may be hired prior to being qualified for professional status. These staff will be given the status of intern until all training is completed and final qualifications are met. They may serve in a paraprofessional capacity under the supervision of a QDDP.
